Los Angeles-based AIDS Healthcare Foundation (AHF), a non-profit organization, is a global organization providing cutting-edge medicine and advocacy regardless of ability to pay to patient’s more than 2 million in 48 countries. AHF has its bureaus in four regions including Africa, Asia, Europe and America. AHF Asia Bureau, is located in Phnom Penh, is operating its activities in a number of countries in Asia such as: Cambodia, China, Indonesia, Myanmar, Nepal, Philippines, Thailand, Vietnam and LAOS.
Currently, AHF Cambodia is seeking for a full time qualified and experienced candidate for the positions of Nurse Counselor based in NCHADS- AHF (ART CLINIC- 3) Phnom Penh.
- Nurse Counselor (1 Position)
- Conduct preliminary interviews/counseling for ART to selected clients so in order to assess client’s immediate needs, including medical, housing, mental health, transportation and other supportive services,
- Conduct individual and group sessions to provide psychosocial support, general HIV/AIDS information and risk reduction activities, HIV one‐0n‐one, and partner counseling and referral services (PCRS) in addition to HIV/AIDS treatment information and treatment adherence counseling,
- Conduct boosted adherence (enhance adherence counseling) to clients who has high viral load and risk to treatment failure.
- Gives basic HIV/AIDS and pre-ART/ ART information and support to new patients, including telling them about the benefits of pre-ART/ ART, overcoming common barriers, and getting a good start with pre-ART/ ART,
- Actively participate in trainings, on‐going skills enhancement trainings,
- Assist in Rapid testing and prevention program activities and events upon request – such as trainings, testing events,
- Assist new patients prevent and handle common pre-ART/ ART concerns,
- Assist program development and clinicians to explore for and identify additional linkage to Care services such as legal help, nutritional support; Identify resources and promote program, follow new strategy boosted integrated active case management (B-IACM) at OD level.
- Serve as liaison to community by conducting presentations on all program activities,
- Assist with case management duties including accompanying client to referral appointments and case conferencing with service providers to ensure optimal service delivery to clients.
- Conduct community outreach to locate clients who are out of care or need support to remain in care,
- Ensure the maintenance of appropriate, accurate, and clear documentation of all outreach, education, case management and group activities – report writing,
- Establish a clear bi‐directional process with medical care providers for the receipt and provision of relevant information on the medical and psychosocial status of enrolled clients,
- Facilitate monthly support groups for HIV/AIDS treatment information, treatment adherence counseling, monitoring, and other strategies to support clients in readiness to begin ARV treatment or maintain maximal adherence to prescribed HIV/AIDS treatment,
- Identify resources and promote program; serve as liaison to community by conducting presentations on all program activities. Participate in community activities such as SEEP, health fairs, special events,
- Measure client’s adherence level to routine medications such as set-in and ART and assess barriers to and supports for adherence,
- Participates in conducting needs assessments and recommending trainings based on gaps identified,
- Periodically reassess adherence level, barriers and supports, and make a record of all findings as well as plans generated during counseling,
- Provide case management services including intake, needs assessment, service planning, service plan implementation, service coordination, monitoring and follow up, reassessment, crisis intervention, and case closure.
- Provide quality emotional and social support to clients, more focus for new case,
- Track and record in ART and other relevant registers and program databases all activities undertaken,
- University graduate in Nursing plus Bachelor Degree in Social Sciences is preferable.
- Five year experience in HIV/AIDS program management and working in hospital/clinic
- A sound knowledge in the basic medical disciplines to allow for accurate counseling and health education and management of clinical problems.
- Exhibit honesty and commitment in assisting AHF Cambodia Cares to achieve its goals.
- Team player, service‐minded and able to work under pressure.
- Should have management, counseling, communication and interpersonal skills.
